MEMBER AGREEMENT AND RELEASE FORM

I agree to fulfill the obligations of membership and to follow the Central Iowa MOA’s by-laws and all laws of the State of Iowa or other relevant jurisdiction relating to the operation of motorcycles or other motor vehicles on the public roadways or other places when acting as a member, officer or representative of Central Iowa MOA during any event or other activity sponsored by or associated with Central Iowa MOA. I understand that my failure to act in accordance with these requirements may serve as grounds to terminate my membership in Central Iowa MOA by the proper authorities of the Club.

MOTORCYCLE INJURY RELEASE OF ALL LIABILITY
THIS IS AN AGREEMENT FOR RELEASE OF LIABILITY. READ IT BEFORE COMPLETING REGISTRATION

Riding a motorcycle is a dangerous activity and presents the risk of serious injury or death. I know and understand the risk of danger to myself and my property while riding a motorcycle. With that knowledge, I accept those risks for myself and hereby release the Central Iowa MOA and its officers, directors or other persons acting in their capacity as members or representatives of Central Iowa MOA from any liability whatsoever for any injury to property or person I may suffer, including crippling injury or death, when engaged in any event organized or sponsored by or associated in any way with Central Iowa MOA or its members. I hereby grant permission to Central Iowa MOA to use photographs and/or video of me at events in publications, news releases, online, and in other communications related to the mission of Central Iowa MOA.  I hereby certify that I have read this Agreement and Release Form in its entirety.  By checking the box below, I indicate that I fully understand it and agree to all its terms and conditions.
